To provide telephonic counseling on Diabetes Management.
To educate and handling queries related to diabetes and associated co-morbid conditions.
To enroll diabetic patients in the Support Program.
To build rapport and relationship with the patient.
To provide customized diet plans to the patient on diabetes and its management.
To create goals for behavior modification to improve standard of living for the patient.
To maintain accuracy and sanctity of data.
